Internet Explorer needs to be restarted to show ads, after disabling the ad blocking.
Steps to reproduce:
1. Make sure you have the ad blocker enabled.
2. Go to a website which has ads.
3. You will not see the ads.
4. Click disable everywhere.
5. Refresh the page. -> the ads are still blocked.
6. Restart browser.
7. Open the website you opened before -> the blocks are visible now.
Second scenario.
Repeat the first scenario, but this time for step 1 you should have ad blocker disabled, and for step 4 you should enable the ad blocker.
The result will be that the ads will not be blocked.
I tested it with IE8.

What i read from your issue is that ABP blocks ads if enabled and doesn't block ads if disabled? If that holds true, I cannot see the issue at all then... If you meant something different, please file a detailed report at https://issues.adblockplus.org
