Filter one element from a whitelisted site

There is an element at x.domain1.com/files/id-string/image.jpg which I have blocked easily. I confirmed that the image does not appear on another website.

That same image is shown on a domain that I have to have whitelisted. I do not want to see it.

I cannot block x.domain.com because I need to see other images and even if I could I assume the whitelisting - which takes the form *.domain2.com/* - would override the domain block.

So am I correct in thinking I am asking too much from Adblock+ ?

Probably it's possible to solve the problem you have on that site without completely whitelisting it. But you have to give us the links to the two sites you are talking about (if you don't want to post them here for whatever reason, you can also send me a PM).
