The ABP icon no longer appears on any page. How do I get it back?

Have you tried, uninstall & reinstall ABP?

I had the same problem and here is what fixed it for me. I have ie9 but might work for all
Go to a blank webpage, or blank area in webpage anywhere. Right Mouse click and click on All Accelerators and then click on Manage Accelerators.
Next click on toolbars and extensions (highlight) When the list comes up highlight Adblock and at the bottom, click on enable. Open a new window it should be there.
If it is there but grayed out, right mouse click on the gray icon and unclick disables..there are two of them.
