When I use the tool to hide an add, it disappeares for a while (according to the 'Preview" option), however the add is back after closing the "Element Hiding Helper" window.

use EHH, create a filter, copy it and post it here. Post also the site where you create the rule.
check also if the filter was added like custom filter
if the filter was written in your custom filters but still has no effect on the page, submit an issue report while on that page and post the report link here (include in the report your addons / extensions)
how to submit an issue report ==>

I dont see in your report any subscription or extensions
adLeaderboard is already hidden using easylist (default subscription)
so, your ABP does not work:
-could be disabled
-check "disable everywhere" option ==> it must be unchecked !
-check your subscriptions ==> click abp icon, filter preferences, see "filter subscriptions" tab ==> if easylist is enabled see to have a succesful downloading, if you see failure go to
http://www.fanboy.co.nz/
point the mouse over easylist (mirror) and choose the list you want to subscribe

Well, I've just used it on an ad I'd tried to hide several times before. Still in vain.

No, ABP is not disabled.

Are you saying that ABP does not work without a "Filter subscription"?
Why not?
I'm happy with my own filters, I really do not want other people to decide what I'll not see ...

yes, you can use your filters, but I dont see adLeaderboard filter in your report
I can see only this filter
You didnt answer all my questions ==> after using EHH what filter did you create ? is that filter written in your custom filters ?

||ad.doubleclick.net^$~object-subrequest,third-party
/admeta.
##.adFull
###adSkyPosition
###adLeaderboard-middle
###adLeaderboard

To be honest, I have no idea what am I supposed to do now. I just used the "Element Hiding Helper" as I had been using all the time before:
- I selected the "Select element to hide" menu item
- I selected an element to hide
- when I saw the element was hidden, I closed the app
In vain, the element was back.
I have no idea, WHAT or WHERE something should be written, I didn't need it before...
However, I've just tried to hide something on google's results:

try creating a new profile
http://kb.mozillazine.org/Creating_a_ne ... on_Windows
then install only ABP and EHH, see if works
then install all other addons and see if still works

Hmm. I'm on Linux, however, point taken.
I'll give it a try ...
